Something like jotform... but not jotform

I like jot form, I like that you can have it to filter out questions that are not relevant depending on prior answers.

However, I want there to be some (5-10) questions at the outset which are answered using yes/no and if the person "passes" and we can help, it will then display a form for them to fill out.

I am also hoping this script can link in with our back office CRM using an API and populate the relevant fields.

I dont suppose there is something that complex out there is there?...
